Happened to me twice. As much details as I know for now:
1. I play Nergal hero (if it even matters).
2. There is some late turn. I have 3-6 cards in hand. Nothing special on board. Possibly big latency on internet connection.
3. I want to use my hero to draw a card. Action is being used, resources are paid. And no card is received.
